胆道与空肠吻合术的改进

Improvement of biliary duct and jejunmn tallying drainage

目的探讨胆道与空肠吻合的手术方式以及远期疗效。方法对2006年10月至2009年10月间施行胆肠袢式吻合和胆肠间隔袢式吻合术27例的病例作回顾性分析。结果27例中11例肝总管横断的病人行胆肠袢式吻合术，16例未横断胆总管的病人行胆肠间隔袢式吻合术，其中26例顺利恢复出院；术后18例获得随访，随访时间12～24个月，均未出现胆系症状再发，疗效满意。结论胆肠袢式或胆肠间隔袢式吻合术，手术操作简单，避免了切断空肠，术后并发症发生率低，值得临床推广应用。

Objective To-dis-cuss the surgical method and the long-term effectiveness of the biliary duct and jejunum tallying drainage. Methods The clinical data of 27 cases treated with the gall- bladder intestine button-fastening loop and the gap button-fastening loop tallying from October 2006 to October 2009 were retrospectively analyzed. Results Eleven cases of common bile duct transection were subjected to the gallbladder intestine button- fastening loop,and the remaining 16 cases received the gap button- fastening loop tallying. Twenty-six cases were cured. After operation, 18 cases were followed up with a period of 12-24 months. No recurrence was found. Conclusion The gallbladder in- testine button-fastening loop or the gap buttonfastening loop tallying are easy and effective,which a voids the jejunum cut-off and has lower incidence of postoperative complications,
