乳牙充填术中四手操作技术的临床研究

A clinical study on four-handed technique in odontoplerosis of deciduous teeth

目的探讨乳牙充填术中四手操作与患儿配合度、椅旁时间及充填质量之间的关系。方法将210例患儿分为传统治疗组（90例）和四手操作组（120例），进行患儿配合度、椅旁时间和材料脱落率比较。结果配合度四手操作组为2．4±0．5，传统治疗组为1．9±0．4；平均椅旁时间四手操作组为20．4±6．2分钟，传统治疗组为34．5±8．4分钟；6、12个月材料脱落率四手操作组为6．7％、8．3％，传统治疗组为20．0％、24．4％。两组3个指标比较有显著性差异（均P〈0．01）。结论四手操作可缩短乳牙充填治疗时间、降低脱落率，提高医生的工作效率和医疗质量，增加患儿的安全感，减少对牙科恐惧症的发生。

Objective To explore relationship between degree of cooperation, chairside time, expulsion rate of filling materials and four- handed technique in odontoplerosis of deciduous teeth. Methods 210 patients were selected for this study and they were divided into four- handed technique group （n = 120） and conventional treatment group （n = 90）. Then the patients were performed on odontoplerosis of deciduous teeth. Degree of cooperation, chairside time and expulsion rate of filling materials of patients in the two groups were recorded and compared respectively. Results The mean score in degree of cooperation among the patients in the four-handed technique group was 2.4 ＋ 0.5, while that in the conventional treatment group was 1.9 -＋ 0.4. The average chairside time of the patients in the four-handed technique group was 20.4 ~ 6.2min, while that in the conventional treatment group was 34.5 -＋ 8.4 min. Expulsion rate of filling materials at 6 and 12 months after treatment in the four-handed technique group were 6.7% and 8.3% respectively, while those in the conventional treatment group were 20.0% and 24.4% respectively. There were significant differences in the three indexes between the two groups （ all P 〈 0.01 ）. Conchlsion In odontoplerosis of deciduous teeth, the four-handed technique can shorten operation time and reduce expulsion rate, improve efficiency and quality of work of the dentists, meanwhile, it can increase children＇s sense of security, lessen their dental phobia.
