肛内超声联合亚甲蓝注射在急性肛管直肠周围脓肿手术中的应用价值

Value of endoanal ultrasonography combined with methylene blue injection in the operation of acute anorectal abscess

目的探讨肛内超声联合亚甲蓝注射在急性肛管直肠周围脓肿手术中的应用价值。方法将确诊为急性肛管直肠周围脓肿的患者138例,合并肛瘘者50例,分为两组,分别在肛内超声、肛内超声联合脓腔内注射亚甲蓝两种方法指导下行手术治疗。结果两组相比,病原学、脓肿分型、手术方式无明显差异,愈合时间存在差异。急性肛管直肠周围脓肿的患者,肛瘘的发病率为36.2%,肛内超声诊断肛瘘及内口的准确率分别为86%、76%。结论肛内超声联合亚甲蓝注射对于肛管直肠周围脓肿的手术治疗具有指导意义,达到了手术彻底性和安全性的统一。

Objective To investigate the value of endoanal ultrasonography combined with methylene blue injection in the operation of acute anorectal abscess.Methods One hundred and thirty-eight patients including 50 with anal fistula diagnosed as anorectal abscess,were divided into 2 groups treated by operation guided by endoanal ultrasonography,endoanal ultrasonography combined with methylene blue injection,respectively.Results There was no statistical significance in etiology,abscess types and operations except cure time between the two groups.The morbidity of anal fistula in patients with acute anorectal abscess was 36.2%,the diagnostic accuracy of anal fistula and internal opening used by endoanal ultrasonography were 86%,76%,respectively.Conclusion Endoanal ultrasonography combined with methylene blue injection is important in the operation of acute anorectal abscess,not only in radical cure,but also in safty.
