CD44蛋白在人喉癌中的表达及其临床意义

EXPRESSION OF CD44v6 PROTEIN INLARYNGEAL SQUAMOUS CELL CARCINOMA

目的：ＣＤ４４基因是一种肿瘤转移相关基因。研究检测ＣＤ４４ｖ６蛋白在喉癌中的表达及其临床意义。方法：用ＣＤ４４ｖ６单克隆抗体，通过免疫组化Ｓ－Ｐ法，对６９例喉鳞状细胞癌蜡块标本和１０例正常喉粘膜标本进行研究，并结合临床资料进行预后分析。结果：６９例喉鳞状细胞癌、１０例正常喉粘膜中ＣＤ４４ｖ６蛋白阳性表达率分别为５２２％（３６／３９）和００％（０／１０）。ＣＤ４４ｖ６蛋白阳性表达与肿瘤发生部位、病理扮级和临床分期无相关性（Ｐ＞００５）；ＣＤ４４ｖ６蛋白阳性表达者颈淋巴结转移率（５０％，１８／３６）比阴性表达者高（１８２％，６／３３），两组有显著性差异（Ｐ＜００１）。用Ｌｏｇ＿ｒａｎｋ检验分析ＣＤ４４ｖ６蛋白表达阴性组生存率高于表达阳性组（Ｐ＜００５）。结论：ＣＤ４４蛋白的阳性表达与喉鳞癌颈淋巴结转移和生存率显著相关，ＣＤ４４ｖ６可能作为颈测喉癌转移和预后的一个重要的指标。

Objective: To examine the immunohistochemical staining of CD44v6 protein in laryngeal squamous cell carcinoma (LSCC) and its clinical significance. Methods: S_P method of immunohistochemistry with CD44v6 monoclonal antibody was employed to study the expression of CD44v6 protein in 69 paraffin_embedded specimens of LSCC and in 10 specimens of normal laryngeal tissues. The clinical pathologic data and results of follow_up were also analysed. Results: In the 69 cases of LSCC,CD44v6 protein positive rate was 52.2% and in the 10 cases of normal tissues the positive rate was 0.0%.The positive expression of CD44v6 protein did not correlate with the position of tumor,and the staging and grade of LSCC(P>005);but the expression of CD44v6 protein was higher in cases with lymph nodal metastasis (50%,18/36) than that in those without lymph nodal metastasis (18.2%,6/33),and the difference was significant (P<001).By Log_rank test,the survival rate of CD44v6 negative expression tumors was higher than that of CD44v6 positive expression tumors (P<0.05). Conclusion: The expression of CDd44v6 is significantly correlated with lymph nodal metastasis and the survival rate.CD44v6 may have value for predicting metastasis potential and prognosis.
