摘要
目的:探讨鼻咽组织、外周血白细胞EBV基因组分布及与NPC的关系。方法:采用PCR法检测26例NPC,4例癌旁及14例慢性鼻咽炎鼻咽组织及外周血白细胞中EBVDNA。同时采用酶免疫法测定血清VCA-IgA。结果:1.NPC、癌旁及鼻咽炎组织EBVDNA无明显差异。2.NPC与鼻咽炎组外周血白细胞EBVDNA有明显差异,P<0.05。3.NPC与鼻咽炎组血清VCA-IgA之间存在明显差异。P<0.05。4.同时检测以上三项后三项阳性率及二项阳性率在两组间有显著差异。P<0.05。5.NPC活检组织及血EBVDNA与血清VCA-IgA阳性率之间有良好的一致性。结论:NPC鼻咽组织存在大量的EBVDNA拷贝数,少数分布于外周血白细胞,且与血清VCA-IgA有良好的一致性。3项套式检测比单项鼻咽活检组织EBVDNA或血清VCA-IgA更有助于对NPC的诊断。
Objective: To study the EBV DNA distribution in the human body
and the relationship between EBV DNA and NPC. Methods: EBV DNA was detected in 26 cases
of NPC, 4 cases of epithelium adjacent to NPC and 14 cases of chronic nasopharyngitis (CNPG)
with polymerase chain reaction (PCR). At the same time, serum VCAIgA was also measured in
NPC and CNPG with enzyme-immunoassay. Results: The positive rate of NPC EBV DNA in the
three groups was not significantly different, but there was a remarkble difference in leucocyte
EBV DNA and serum VCAIgA between NPC and CNPG(P<0.05). It was also found that the positive
rates of multiparameters including serum VCAIgA and EBV DNA both in leucocytes and NPE in
NPC were higher than those in CNPG and there was a good consistency between EBV DNA in
NPC or serum leucocyte and serum VCAIgA. But NPE EBV DNA was not relative to the stage and
lymph node involvement in NPC. Conclusion: There are many EBV DNA copies in NPE or
leucocytes of NPC and there is also a good consistency between EBV DNA in NPE or leucocyte
and serum VCAIgA. So it will be more helpful to NPC diagnosis if a combination of the above
three assays is taken.
