晚期复治消化系肿瘤腹水的腹部透热化疗临床观察 被引量:7

Intraperitoneal chemotherapy combined with whole body hyperthermia by using micro-wave on abodeman for treating malignant peritoneal effusion caused by digest system tumors

摘要 目的:探讨腹部透热全身热疗系统在晚期复治性原发性肝癌、胃肠道癌及间质肿瘤恶性腹水腹腔化疗治疗中的辅助及增效治疗作用价值及意义。方法:试验组(18例)抽放恶性腹水后腹腔内灌注DDP+5-FU主药物后即刻行高功率微波腹部透热全身热疗,恒温期温度41-42℃,维持〉60 min。对照组(24例)仅行腹腔灌注化疗,用药方法同试验组。结果:42例复治恶性腹水总缓解率57.1%(24/42),其中试验组77.8%(14/18),对照组41.7%(10/24),前者有效率明显高于后者,差异有统计学意义,P〈0.05。试验组及对照组患者中位生存时间分别为30和25周。2组不同治疗方法的毒副反应发生率及程度差异无统计学意义,P〉0.05。结论:以腹腔灌注化疗联合经腹透热全身热疗治疗晚期肿瘤恶性腹水较常规单纯腹腔化疗疗效好,且毒副反应少,并有一定程度延长患者生存期作用,有临床实际应用价值。 OBJECTIVE:To study the adjuvant effective valuation and significant of whole body hyperthermia by using micro-wave on abodemn in intraperitoneal chemotherapy for malignant peritoneal effusion caused by digest system tumors.METHODS: Totally 18 cases was performed by intraperitoneal infusion DDP and 5-Fu combated with whole body hyperthermia,temperature 41-42 ℃,continue for more than 1 hours as treated group.Totally 24 cases was treated by intraperitoneal infusion chemotherapy alone as the control.RESULTS: The response rate(RR) in all 42 cases was 57.1%(24/42),in which the group of intraperitoneal infusion chemotherapy combined body hyperthermia was 77.8%(14/18),the control group was 41.7%(10/24),and the difference was significant(P0.05).The mid-survival times(MST) in treated group and control were 30 and 25 weeks.The side-effect was no significance different between two groups(P0.05).CONCLUSIONS: Combinting intraperitoneal infusion chemotherapy and whole body thermotherapy by using micro-wave on abdomen are more effective management in increasing RR of malignant in peritoneal effusion and prolong survival time than that of intraperitoneal chemotherapy alone.There are clinical application value by using the method of body hyperthermia.
