左旋肉碱对维持性血液透析患者左心室肥厚的影响

摘要 目的观察终末期肾脏病(ESRD)接受维持性血液透析(MHD)治疗的患者心脏结构、功能变化;探讨左旋肉碱对MHD患者左心室肥厚的影响。方法选取ESRD接受MHI)患者57例,根据是否应用左旋内碱分为A组(左旋肉碱纽。26例):B组(非左旋肉碱组,31例)。,观察两组患者左心房收缩末期内径(IADs)、左心室舒张末期内径(LVDD)、室间隔舒张末期厚度(IVST)、左心室后壁舒张末期厚度(LVPWT)、二尖瓣前向血流频谱舒张早期与心房收缩期左室充盈峰速度比值(FJA)、射血分数(EF)、颈内动脉内中膜厚度(IMT),计算左心室心肌重量(LVW)和左心室心肌重量指数(LVWI),进行统计学分析。结果干预6个月后两组LADS.INDD,IVST,LVPWT,E/A,LVM,LVMI,EF比较,差异有统计学意义。结论左旋肉碱对维持性血液透析患者左心室肥厚进展有延缓作用. Objective To observe the cardiac structural and functional change in patients with the end-stage renal disease who receiving maintenance hemodialysis and to explore the influence of L-Carnitine on left ventricular hypertrophy. Methods Fifty-seven patients with end-stage renal disease who receiving maintenance hemodialysis were selected. They were divided into group A ( reeeiving L-Carnitine therapy,n = 26) and group B( no L-Carnitine,n = 31 ). To detect the left atrium eontractinn last stage inside diameter( LADS} ,left ventricle diastole last stage inside diameter ( LVDD} ,interventricular septem diastole last stage thickness(IVST) ,left ventricle behind diastole last stage thickness(LVPWT) ,bicuspid valve forward blood stream frequeney spectrum diastole early time and atrium systole left room full peak speed ratio( E/A} ,ejecting fraction(EF) and inner-media membranes thickness of inner carotid artery(IMT). Left ventricle cardiac muscle weight(LVW) and left ventricle cardiac muscle weight index(LVWI) were counted in the two groups. Results There was statistical difference in LADS, LVDD, IVST, LVPWT,E/A, EF, LVW anti LVWI between group A and B six months later. Conclusion L-Carnitine has postponing effect on left ventrieular hypertrophy in patients during mainting hemodialysis.
