食管癌切除术后并发幽门梗阻的诊断和治疗(附9例报告)

Diagnosis and Treatment of Pyloric Obstruction after Excision of Esophageal Cancer

目的：报告食管癌切除术后并发幽门梗阻９ 例的诊断与治疗。方法：再次手术治愈。结果：男性７ 例，女性２ 例，梗阻发生时间在术后５ ～９ 天，９ 例全部经再次手术治愈。其中幽门扭转３ 例；胃大弯侧膈下部分褶叠扭转１ 例；十二指肠球部周围纤维粘连带２ 例；胃上提过高，幽门附近粘连压迫２ 例；胰腺部位肿瘤压迫１ 例。结论：对此并发症的发生原因、诊断、治疗和预防进行了讨论。

Nine cases (male 7, female 2) of pyloric obstruction after esophagectomy for esophageal cancer were treated by operation. The obstructions occurred during the period of 5～9 days postoperatively. It was found that 3 cases were caused by volvulated pylorus,1 subphrenic folded torsion of greater curvature, 2 periduodeno-bulbar adhesive band, 2 adhesive compression to the pylorus due to over-elevation of the stomach, and one case by compression of a pancreatic tumor. The etiology, diagnosis, treatment and prevention of this complication were discussed.
