经颅彩超应用于新生儿缺血缺氧性脑病的检测 被引量:3

摘要 目的:运用经颅彩色多普勒超声(Transcranial Doppler,TCD)检测缺氧缺血性脑病(Hypoxic Ischemic encephalopathy,HIE)及正常新生儿脑血流动力学变化,评估及预测缺血缺氧性脑病患儿的病情及预后。方法:对2010年4月-2010年8月期间共45例临床诊断为缺血缺氧性脑病患儿及20例正常新生儿分组进行比较,对其TCD检查结果进行对比分析。结果:缺血缺氧性脑病患儿大脑动脉的收缩期峰值流速S可升高或降低,舒张末期峰值流速D降低明显,S/D升高,阻力指数RI明显增高,尤其以大脑中动脉和大脑前动脉较敏感。45例缺血缺氧性脑病患儿中大脑动脉的舒张末期峰值流速较高(D〉10cm/s),S/D较低(〈3.0),RI较低(〈0.63)的患儿预后较好。结论:运用TCD检测新生儿大脑动脉的血流情况,分析血流速度,S/D比值,RI值,对评估及预测缺血缺氧性脑病患儿的病情及预后有重要的参考价值。 Goal: The utilization supersonic (Transeranial Doppler, TCD) examines the oxygen deficit anemic brain illness after skull colored Doppler (Hypoxic Ischemic encephalopathy, HIE) and the normal newborn brain blood stream dynamics change, the appraisal and the forecast ischemia anoxic brain illness babys condition and the prognosis. Method: Period altogether 45 example clinical diagnoses group to April, 2010 - the August, 2010 for the ischemia anoxic brain illness baby and 20 example normal newborns carry on the comparison, carries on the contrastive analy- sis to its TCD check result. Finally: Ischemia anoxic brain illness baby cerebrum arteryg systole peak value speed of flow S may elevate or reduce, diastole last stage peak value speed of flow D reduces obviously, S/D elevates, the drag index RI obvious markup, especially the artery is sensitive before the cerebrum median artery and the cerebrum, in 45 example ischemia anoxic brain illness baby the cerebrum arteryg diastole last stage peak value speed of flow is high (D 〉 lOcm/s) , S/D is low ( 〈3.0), RI is low (〈0.63) the baby prognosis is good. Conclusion: Examines the new- born cerebrum arteryg blood stream situation using TCD, the analysis blood stream speed, the S/D ratio, the RI value, to appraises and the forecast ischemia anoxic brain illness babyg condition and the prognosis has the important reference value.
