游离椎弓根峡部嵌入植骨术治疗合并椎间盘突出的腰椎弓峡部裂 被引量:2

摘要 目的:介绍一种治疗合并有椎间盘突出的腰椎椎弓峡部裂的手术方法。方法:先将患椎的后半部分游离取下并修整,待椎间盘切除、植骨床修整后,取髂骨块嵌入,再将后半部分回植,用螺丝钉贯穿固定,同时行横突椎板植骨。结果:本组治疗13例,其中11例随访1年以上,临床症状消失,峡部裂均融合,患椎滑脱无继续加重。结论:该术式操作简单,确实可靠,疗效理想。 Objective:To introduce free lumbar isthmic inlay bone grafting for treatment of lumbar disc herniation and spondylolysis.Methods:The first step was to take out the back part of affected vertebral arch and trimmed it.After herniated disc was removed,the bed for bone graftig in isthmic part was also trimmed,put an inlay graft of iliac bone.Then the back part of vertebrae was reimplanted and fixed by two screws,at last,bone grafting for fusion between lamina and transverse process was performed.Results:This technique was used in 13 cases.There were 11 patients followed-up more than one year.The results were the clinical symptoms disappeared,spondylolysis gained bone fusion and spondylolisthesis did cured.Conclusions:The operation had the advantages of less operative trauma,simple and stable internal fixation.The curative effect was excellent.
