自行调节式口腔矫治器治疗OSAHS患者舌咽部的形态学改变 被引量:1

A Morphology Study on the Change of Glossopharyngeal Part in OSAHS Patients Treated by Elastic Dental Appliances

摘要 目的研究阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者经自行调节式口腔矫治器治疗后舌咽部变化,为临床上口腔矫治器治疗OSAHS患者提供理论依据。方法 12例OSAHS患者口腔矫治器前后分别摄X线头颅定位侧位片和上气道螺旋CT,分别测量H-RGn、H-CVP、AH-CC、H⊥S、PAS、AH⊥MP、AH⊥C3Me和舌咽部左右径、前后径和横截面积,比较舌咽部相关组织的位移变化。结果头影测量发现PAS、AH-CC和AH⊥C3Me明显增加,AH-RGn、H⊥S和AH⊥MP减小,均有统计学意义(P<0.05或P<0.01);CT发现,戴入口腔矫治器后,上气道舌咽部左右径和横截面积明显增加,有统计学意义(P<0.05)。结论自行调节式口腔矫治器能改变舌骨的位置,减轻舌咽部平面的阻塞,扩大上气道间隙。 Objective To investigate the change of glossopharyngeal part in OSAHS patients cured by elastic dental appliances and to supply the scientific data for treating obstructive sleep apnea hypopnea syndrome with oral appliances.Methods 12 patients with OSAHS received elastic dental appliances treatment.Before and after operation,cephalometric radiographs and CT were obtained and compared.Results H-RGn,PAS,AH-CC,AH⊥C3Me,H⊥S,AH⊥MP parameters showed significant changes after dental appliances inserted.Before and after CT compared,coronary axes and cross section of glossopharyngeal part were increased significantly.Conclusion After the position of hyoid bone was change by elastic dental appliances,the obstruction of glossopharyngeal part was relieved and the upper airway spaces were enlarged.Lateral cephalometric radiographs may reflect the changes of the glossopharyngeal part of OSAHS.
