医院合理用药管理模式评价

Management mode of rational drug use in hospital

目的评价以药学专业知识为基础的合理用药管理工作模式。方法建立以副主任药师为主、具有行政管理职能的合理用药办公室,根据医院不合理用药相对集中的品种,将抗菌药、中成药注射剂作为重点监管品种,制定合理用药判定标准、考核办法及教育体系,并将合理用药相关指标纳入医院考核体系,以实施干预与否住院患者均次药费,手术预防用抗菌药物的合理、规范使用率,以及中成药注射剂规范使用率等对临床合理用药情况进行评估。结果住院患者均次药费降低,手术预防用抗菌药物的合理、规范使用率提高,中成药注射剂规范使用率提高。结论建立以药学专业知识为基础的长效化、系统化行政管理科室模式,对临床不合理用药进行有效干预,可充分发挥临床药师的作用,有利于提高临床合理用药水平。

Objective To evaluate the management mode of rational drug use base on the pharmaceutical knowledge. Methods An established ＂rational drug use office＂ with administrative function was directed by associate chief pharmacist. According to the cases of irrational drug use in our hospital, especially on antibacterial drugs and Chinese traditional medicine injection, the office made a management program to inspect the prescription, to set up rational drug use standard, assessment standard, education, and the related indexes of rational drug use. Clinical rational drug use condition was evaluated by checking in hospital patient＇s average cost of medicine, the rational usage rate of prophylactic antibiotics and Chinese traditional medicine injection. Results In hospital patient＇s average cost of medicine reduced, the rational usage rate of prophylactic antibiotics increased and the standard usage rate of Chinese traditional medicine injection increased. Conclusion The managment mode of establish the administration department based on the pharmacy professional knowledge can provide the basis for the effective intervention of irrational drug use, improve the rational use of drugs. Thus clinical pharmacist will really play a more important role to improve hospital＇s medical service quality.
