Treatment of comminuted patellar fracture using patellar rings

摘要 目的评估髌骨环治疗髌骨粉碎性骨折的治疗结果。方法对31名髌骨粉碎性骨折的病人应用髌骨环内固定,术后对病人进行至少1年的随访,搜集数据主要包括术后骨折愈合时间,疼痛程度,膝关节的活动范围,并且记录Bstman评分及术后并发症。结果骨折平均愈合时间为术后3个月,没有骨折再移位和内固定失效的发生。根据VAS(Visual Ana-logue Scale/Score)疼痛评分,术后1年取出内固定后膝关节平均得分为0.3,Bstman平均得达到28.9分。术后膝关节的活动度平均为130.4°,和健侧相比差异无统计学意义(P>0.05)。结论应用髌骨环治疗髌骨粉碎性骨折可以获得良好的临床结果,不增加并发症的发生,有利于患膝早期功能锻炼。 Objective To evaluate a new fixation technique for comminuted patellar fracture using patellar rings.Methods A total of 31 patients(average: 54.8 years) with comminuted patellar fractures were treated by patellar rings.Data of bone union time after surgery,Bstman score at time of bone union,degree of pain on a visual analogue scale(VAS),range of motion of the knee at time of bone union,and postoperative complications were recorded.Results Bone union was achieved at 3rd month after surgery.No cases had re-dislocation and failure of internal fixation.At 12th month after surgery,mean Bstman score was 27.9,knee motion was 130.4° and visual analogue pain score was 0.3,No statistically significant difference was found in knee flexibility between affected and uninjured limb at 12th month after surgery(P〉0.05).Conclusion The new fixation technique using patellar rings resulted in good outcomes and is beneficial for patient to start early functional exercise.
