自制颈椎侧块锚钉在单开门颈椎管扩大成形术中的应用 被引量:1

摘要 目的评价自制颈椎侧块锚钉在单开门颈椎管成型术(expansive open-door laminoplasty,ELAP)中的临床应用效果。方法对28例需行单开门颈椎管扩大成形术的患者,术中应用自制颈椎侧块锚钉进行"门轴"侧固定,利用JOA评分及影像学检查进行临床疗效评价。结果本组患者在术中应用自制颈椎侧块锚钉固定时均未出现神经、血管损伤等并发症,术后随访6-18个月,平均12个月,术前平均JOA评分8.4分,随访时平均JOA评分14.2,平均增长率为65.1%;影像学评估显示内固定确实,未见螺钉松动和再"关门"现象。结论在颈后路单开门颈椎管扩大成形术中应用自制颈椎侧块锚钉固定操作简单,临床效果良好。 Objective To evaluate the clinical results of expansive open-door laminoplasty using the self-designed cervical lateral mass anchor fixation.Methods Twenty-eight patients were subjected to expansive open-door laminoplasty(ELAP) by the self-designed cervical lateral mass anchor.The JOA scores and imaging data were used to evaluate clinical results of these cases.Results There was no neurological and vertebral artery injury after the self-designed cervical lateral mass anchor fixation.The mean follow-up was 12 months(6-18 months),the average preoperative JOA scores were 8.4 and postoperative JOA scores were 14.2,the average improvement rate was 65.1%.The imaging data evaluation showed there was no screw loosening and reclose of the opened laminae.Conclusion The unilateral open-door laminoplasty using the self-designed cervical lateral mass anchor fixation has the advantage of easy manipulation,rigid fixation of elevated laminae and good clinical results.
