双极人工股骨头置换术治疗高龄股骨颈骨折临床观察

Clinical observation of bipolar artificial femoral head replacement for the treatment of fracture of femoral head in elderly patients

目的观察双极人工股骨头置换术治疗高龄股骨颈骨折的临床疗效。方法对73例患者采用双极人工股骨头置换术治疗,其中单骨髋70例,双髋3例。记录手术时间、出血量与治疗前后1周,1月,3月,6月,Harris评分,观察功能恢复与并发症发生情况。结果双极人工股骨头置换术手术时间短,出血量少,术后1个月能获得满意的髋关节功能,并发症发生率低,术后1个月髋关节功能恢复良好。术后10天Harris评分较术前明显升高,差异有显著性意义(P<0.01),术后1个月Harris评分较术后1周升高,差异有显著性意义(P<0.05)。结论对于年龄在70岁以上股骨颈骨折的患者采用双极人工股骨头置换术治疗,能获行良好的短期疗效,未见重大并发症发生,值得推广。

Objective To observe the clinical efficacy of bipolar artificial femoral head replacement for the treatment of fracture of femoral head in elderly patients.Methods From January 2004 to June 2010,73 cases with fracture of femoral head underwent bipolar artificial femoral head replacement.Among them,the unilateral hip operation was in 70 cases and bilateral hip operation was in 3 cases.The operative time,bleeding volume,Harris score in preoperative and postoperative 1 week,1、3 、6-month function of joint and postoperative complications were recorded.Results This procedure had a shorter operative time,less blood loss,few postoperative complications and a satisfactory recovery of auricular function was got in one month after operation.Harris score in 10 days after operation was higher than that pre-operation and there was significant difference（P0.01）.Harris score in 1 month after operation was higher than that 1 week after operation and there was significant difference（P0.01）.Conclusion Bipolar artificial femoral head replacement possesses a good efficacy of recent period and no severe postoperative complications for the treatment of fracture of femoral head in elderly patients.
