铜绿假单胞菌多重耐药株β-内酰胺酶的检测与分析 被引量:3

摘要 目的了解临床铜绿假单胞菌对14种抗生素的耐药情况,对多重耐药铜绿假单胞菌进行β-内酰胺酶检测,为临床合理使用抗生素提供参考。方法 236株铜绿假单胞菌采用K-B法进行药敏实验,筛选多重耐药铜绿假单胞菌后,再进行改良三维实验对多重耐药菌所产β-内酰胺酶进行分析。结果在体外药敏实验中,铜绿假单胞菌对临床常用14种抗假单胞菌药物均存在不同程度耐药现象,氨基糖苷类及喹诺酮类抗生素显示出较高的敏感性,分别为环丙沙星(60.6%)、加替沙星(58.1%)、阿米卡星(43.6%)、左氧氟沙星(53.4%);其次是亚胺培南(56.4%)、头孢他啶(49.2%)、头孢哌酮/舒巴坦(40.7%)。三维实验结果显示,产超广谱β-内酰胺酶(ESBLs)菌8株(25.0%),产头孢菌素酶(AmpC酶)菌7株(21.8%),同时产ESBLs+AmpC酶菌4株(12.5%)。乙二胺四乙酸(EDTA)协同实验检测产金属酶菌5株(15.6%)。结论产ESBLs、高产AmpCβ2内酰胺酶和(或)碳青酶烯酶是本院多重耐药铜绿假单菌主要耐药机制。
