长期留置双腔导管在血液透析中的应用及护理

Application of long-term indwelling double tube catheter in hemodialysis and nursing experience

目的:探讨长期留置双腔导管在血液透析中的应用及护理。方法:对我院血透室应用长期留置双腔导管行血液透析20例患者的临床护理资料进行回顾性分析。结果:本组20例患者,留置导管时间9～48个月,血液流量平均180～250 mL/min,其中1例曾出现隧道口皮肤红、肿、热、痛反应,经局部换药处理及加强全身抗感染治疗后,炎症消除,可继续血液透析。结论:长期留置双腔导管操作简便,并发症少,并能提供充足的血流量,适合于多种原因导致动静脉内瘘建立困难的患者。配合以正确的使用方法及细致的护理,严格无菌操作,可减少导管并发症,延长导管的使用寿命,提高患者的生活质量。

Objective：To discus the application of long-term indwelling double tube catheter in hemodialysis and its nursing care.Methods：The clinical data in 20 cases of hemodialysis with long term indwelling double tube catheter were retrospectively analyzed.Results：Among 20 cases in this group,the indwelling catheter time was 9-48 months and the average blood flow rate was 180~250mL/min.1 case once presented the tunnel portal skin red,swell,hot and painful reactions,which inflammations were eliminated and hemodialysis was continued after local change of dressing and strengthening the systemic anti-infection treatment.Conclusion：The long-term indwelling double tube catheter is simple in operating with few complications,which can provide the sufficient blood stream quantity,suitable for the patient with difficult establishment of internal arteriovenous fistula due to many kinds of reasons.The coordination by the correct application method and careful nursing as well as the strict aseptic technique,may reduce the tube-induced complications and lengthen the tube＇s service life,so as to improve patient＇s quality of life.
