Activation of epidermal growth factor receptor (EGFR) leads to unrestrained oncogenic signaling and occurs via distinct mechanisms in multiple human cancers. Certain somatic kinase domain mutations, such as those that occur in non-small cell lung cancer, confer sensitivity to EGFR inhibitors. In contrast, a mutational spectrum mainly exemplified by extracellular domain deletions is found in glioblastoma multiforme, which is also characterized by aberrant EGFR signaling but has proven largely non-responsive to therapeutic targeting of EGFR. Extending the utility of EGFR-targeted agents for the treatment of human cancers is dependent upon an increased understanding of how the underlying differences in EGFR dictate clinical response and resistance.
