Clinical Significance of sCD40L Level After Percataneous Coronary Intervention in Patients with Acute Myocardial Infarction

目的:探讨急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)前后血清可溶性CD40L的水平变化及临床意义。方法:应用酶联免疫吸附法对40例AMI患者进行了PCI治疗前后血清sCD40L、cTnI及CK-MB的检测,并与40例对照组患者进行比较。结果:AMI组sCD40L水平在PCI术前及术后即刻及术后2h均高于对照组(P<0.05),组内比较,AMI组sCD40L在术后即刻明显增高,较术前有显著性差异(P<0.05);PCI术前、术后即刻及术后2h,sCD40L阳性检出率分别显著高于cTnI和CK-MB;AMI死亡组sCD40L在PCI术前、术后即刻及2h、6h、24h均明显高于AMI存活组。结论:sCD40L在AMI患者PCI治疗前后的早期诊断和预后评估均有重要的临床价值。

Objective To study the clinical significance of changes of plasma soluble CD40 ligand（sCD40L） level after percataneous coronary intervention（PCI） in patients with acute myocardial infarction.Methods Plasma sCD40L,cTnI and creatine kinase（CK）-MB levels were determined in 40 patients with acute myocardial infarction（AMI） both before and after PCI as well as in 40 controls.Results The level of sCD40L of AMI group was higher than control group at baseline and instantly,2h after PCI（P0.05）,in the same group（AMI group）,the level of sCD40L was significantly higher at instantly after PCI than at baseline（P0.05）,the positive rate of sCD40L was notably higher than that of cTnI and CK-MB at baseline and instantly,2h after PCI,thelevels of sCD40L in AMI death group were prominently higher than that of AMI live group at baseline and instantly,2h,6h and 24h after PCI.Conclusion Plasma sCD40L level had important clinical value in patients with AMI for early diagnosis and outcome both before and after PCI.
