Ⅱ类切口手术术前预防用抗菌药物给药时机及影响因素调查

Investigation of Medication Time and Influencing Factors of Preoperative Prophylactic Application of Antibiotics in Type Ⅱ Incision Surgery

目的:探讨Ⅱ类切口(清洁-污染切口)手术患者皮肤切开前抗菌药物的给药情况及主要影响因素。方法:在我院手术室随机观察Ⅱ类手术患者切开皮肤前抗菌药物的给药情况,分析存在的问题。结果:在观察用药的118例患者中,有61例由手术室护士于切开皮肤前30min内按常规顺序给药,50例经干预于切开皮肤前30～60min内给药,7例未用药。切开皮肤前60min内用药率达94.06%。影响给药时间的因素包括皮试结果、手术开始时间不确定、医嘱问题、药品错误及交接班不清。结论:确保术前预防性抗菌药物正确应用,需加强相关人员的协作和配合。

OBJECTIVE：To explore prophylactic application of antibiotics before type Ⅱ incision surgery （clean-contaminated incision） and identify the major influencing factors of it.METHODS：Prophylactic application of antibiotics in patients undergoing type Ⅱ incision surgery in operation room of our hospital was observed to analyze the problems.RESULTS：Of observed 118 cases,61 cases were given medicine by nurses within 30 min before incision,while 50 cases were given medicine within 30～60 min before incision and 7 cases didn＇t use any kind of antibiotics.The rate of prophylactic application of antibiotics within 60 minutes before incision was 94.06%.Relevant influencing factors included dermal test,operation duration,the link of prescription,medicine taking and handover.CONCLUSION：It is necessary to strengthen cooperation and coordination with relevant personnel in surgical environment to ensure appropriate prophylactic application of antibiotics.
