CT Analysis of lntraabdominal Extra Gastrointestinal Stromal Tumors

[目的]总结腹内胃肠道外间质瘤（EGIST）病例的临床和影像学表现,以增加对EGIST的认识,提高诊断正确率.[方法]回顾性分析本院27例有完整手术病理结果及CT检查的腹内EGIST病例临床资料.[结果] 27例中良性6例,交界性4例,恶性17例.其中来源于小肠系膜18例、大网膜5例及小网膜2例、腹膜后2例.EGIST病灶多较大,密度不均.增强扫描良性病灶边界较清,强化较均匀.交界性及恶性肿块多边界欠清,边缘强化,其中心坏死区无强化改变.9例病灶有多发转移,2例病灶为多发肿块.[结论]腹内EGIST具有较典型的影像学特征,CT检查可对其准确定位,对其良恶性判断亦很有价值.熟练掌握EGIST的影像学表现,对其临床诊断和治疗很有帮助.

[Objective]To summarize the clinical and CT manifestations of extra gastrointestinal stromal tumors（EGIST） in order to improve the cognition of EGIST and increase the diagnostic accuracy. [Methods] Clinical data of 27 cases of EGIST with complete surgical pathological results and CT findings were analyzed retrospectively. [Results]Among 27 cases, 6 cases were benign, and 4 cases were borderline, and 17 cases were malignant. The tumors originated from the mesentery（ n = 18）, omentum majus（ n =5）, omentum mi nus（ n =2） and retroperitoneum（ n = 2）. The lesions were usually large and uneven. Enhancement scanning showed that the benign lesions had usually clear margin and homogeneous enhancement, and the borderline or malignant lesions had usually unclear margin and peripheral enhancement which the central necrosis had no en- hancement. Multiple metastases were found in 9 cases. The lesions of 2 cases were nmltiple tumors. [Conclu sionlIntraabdominal EGIST has typical characteristics of CT findings. CT scan can precisely localize EGIST and is valuable in differentiating benign and malignant EGIST. Mastering skillfully these manifestations is helpful for the diagnosis of EGIST.
