摘要 目的研究CO2激光应用于肾脏手术的可行性。方法实验以家兔为对象,按照完全随机的方法将家兔肾脏分组,每组接受不同参数照射。照射后量取消融弹坑的直径和深度,并通过肉眼观测、组织学检查及统计分析等方式研究CO2激光对兔肾实质作用的生物学特性。结果 CO2激光对肾实质有良好的汽化、切割及凝固效果,同时止血迅速。通过统计分析得出其消融弹坑直径与单次脉冲能量、频率及照射时间都具有良好的相关性。照射后消融弹坑的直径和深度随能量密度的增加而增大,当能量密度较小时,增长的速度较快,随着能量密度的增大增长的趋势逐渐减慢,最终趋于平缓。结论只要采取适宜的剂量参数,CO2激光对肾脏的非接触照射是安全、有效的。 Objective To explore the feasibility and clinical significance of CO2 laser irradiation for partial nephrectomy.Methods Fourteen rabbits for animal test were divided into groups randomly,whose kidneys were irradiated with CO2 laser in vivo.Each kidney group was treated with certain parameters of CO2 laser,including pulse energy,frequency,and irradiation time.The depth and diameter of the crater were measured immediately after irradiation.The biological effects were observed with naked eyes and histological examination,and were analyzed statistically.Results CO2 laser had good vaporization,cutting and coagulation effect on the kidney and could stop bleeding rapidly.There was a positive correlation between the depth of crater and pulse energy,frequency and irradiation time.The depth and diameter of the crater increased with the energy density.With the increase of the energy density,the increase trend gradually slowed down and eventually stopped.Conclusions With appropriate dosage parameters,the non-contact radiation of CO2 laser on kidney is safe and effective in treatment.
