颅底硬膜外表皮样囊肿恶变1例报告并文献复习

Malignant transformation of epidural epidermoid cyst in skull base:a case report and literature review

目的探讨颅底硬膜外表皮样囊肿恶变的临床特征及预后。方法报告颅底硬膜外表皮样囊肿恶变1例,结合相关文献分析其临床、影像学及病理特点,探讨其诊断、治疗及预后。结果颅底硬膜外表皮样囊肿恶变临床比较少见。影像学对表皮样囊肿典型病例诊断不难,恶变病例较困难,常需术后病理。外科治疗为首选。结论表皮样囊肿恶变机制目前不清,治疗应在手术切除基础上给予放射和化学治疗,预后欠佳。

Objective To study the dinical feature and prognosis of the malignant epidural epidermoid cyst.Methods The imaging feathures were analysed in a patient with malignant epidural epidermoid cyst in skull base.The radiological and pathologic findings were reviewed in the literatures related to malignant epidermoid cyst.Results The malignant transformation of epidural epidermoid cyst in skull base was very rare.Imaging diagnosis for the typical epidermoid cysts was not difficult,but difficult in the malignant ones,often need postoperative pathologic.Surgical treatment was preferred.Conclusion The mechanism of malignant transformation of epidermoid cyst is not clear.Radiation and chemotherapy should be given on the basis of the surgical resection,but the prognosis is bad.
