摘要 目的探讨凋亡抑制因子Survivin及凋亡促进因子Smac在卵巢上皮性肿瘤中的表达及临床意义,并分析二者的相关性。方法 RT-PCR法检测20例上皮性卵巢癌及20例良性卵巢上皮性肿瘤组织中Survivin及Smac基因的表达情况;免疫组化S-P法检测60例上皮性卵巢癌及60例良性卵巢上皮性肿瘤组织中Survivin及Smac蛋白的表达情况,分析其与上皮性卵巢癌患者各临床病理指标的关系及两者表达的相关性。结果上皮性卵巢癌组织中Survivin基因表达水平(70.0%)明显高于良性卵巢上皮性肿瘤组织(5%),Survivin蛋白在上皮性卵巢癌组织中的阳性表达率为61.7%,而在良性卵巢上皮性肿瘤组织表达率6.7%,免疫组化结果与RT-PCR结果一致;Survivin蛋白在上皮性卵巢癌组织中的阳性表达率与浸润深度、分化程度及淋巴结转移均相关(P<0.05)。RT-PCR结果显示,Smac基因在上皮性卵巢癌组织中的阳性表达率为40%(8/20),明显低于良性卵巢上皮性肿瘤组织90%(18/20),此结果得到免疫组化的证实,Smac蛋白在上皮性卵巢癌组织中的阳性表达率为41.7%,明显低于良性卵巢上皮性肿瘤组织93.3%,而且Smac蛋白在上皮性卵巢癌中的阳性表达率与组织分化程度、浸润深度及淋巴结转移密切相关(P<0.05)。Survivin蛋白与Smac在上皮性卵巢癌癌组织中的表达呈明显负相关(P<0.05)。结论抗凋亡蛋白Survivin和促凋亡蛋白Smac之间存在着相互拮抗,相互调节的关系,在卵巢上皮性肿瘤的发生发展中共同发挥作用。 Objective To detect the expressions of Survivin and Smac mRNA and protein in epithelial ovarian tumor tissues,to investigate the clinical significance and analyze their correlation.Methods The Survivin and Smac mRNA expression levels from 20 cases of epithelial ovarian cancer and 20 cases of benign epithelial ovarian tumors were quantified by semi-quantitiative RT-PCR,and immunohistochemical staining(S-P method) was used to detect Survivin and Smac protein expressions in 60 cases with epithelial ovarian cancer and 60 cases with benign epithelial ovarian tumor.The relationship between Survivin and Smac protein expression was explored,and their relationship with relevant clinical pathologic characteristics were studied.Results The up-regulation of Survivin gene was observed in 70.0%(14/20) epithelial ovarian cancer tissues in comparison with the paired benign epithelial ovarian tumor tissues.Positive staining rate for Survivin was 61.67% in epithelial ovarian cancer tissues,while 6.7% positive staining in benign epithelial ovarian tumor tissues was(P0.05),and the expression of Survivin in epithelial ovarian cancer tissues was positively correlated with invasion depth,lymph node metastasis and differentiation(P0.05).Down-regulation of Smac gene was quantified in epithelial ovarian cancer tissues(40%) in comparison with the paired benign epithelial ovarian tumor tissues(90%).Positive staining rate for Smac was only 41.7% in epithelial ovarian cancer tissues,while 93.3% in benign epithelial ovarian tumor tissues(P0.05).The expression level of Smac was related to invasion depth,lymph node metastasis and differentiation(P0.05).Negative relationship was found between Survivin and Smac in epithelial ovarian cancer tissues(P0.05).Conclusion The expression of anti-apoptosis protein Survivin was significantly correlated with pro-apoptosis protein Smac,and both of them might play an important role in the occurrence and development of epithelial ovarian tumor.
