心理治疗对保守治疗腰椎间盘突出症的疗效影响

Influence of psychotherapy on the effect of conservative treatment for protrasion of the lumbar intervertebral disci

目的 探讨心理治疗对保守治疗腰椎间盘突出症病人治疗效果的影响.方法 将我院诊及住院符合腰椎间]盘突出症诊断标准的患者72例,随机分成治疗组和对照组,每组36例,对照组采用保守治疗;治疗组在保守治疗的基础上,针对患者的心理障碍情况进行心理治疗,随访3个月,比较两组腰痛评分与治疗有效率等方面.结果 治疗后第3、7、14天治疗组腰痛缓解评分（VAS）分别为（5.17±0.71）分、（4.83±0.74）分、（3.52±0.82）分;比对照组的（6.13±0.75）分、（5.97±0.76）分、（5.24±0.85）分明显降低（P＜0.05）,表明治疗组缓解症状方面明显优于对照组;治疗后1、3个月评价有效率分别为91.7%、88.9%与对照组的有效率80.6%、75.0%,比较两组差异有显著性（P＜0.05）,提示治疗组的疗效明显优于对照组.结论 心理治疗能影响保守治疗腰椎间盘突出症的疗效,在保守治疗的基础上协同心理治疗,可以明显缓解腰痛,提高治疗有效率.

Objective To observe the influence of psychotherapy on the effect of conservative treatment for patients with protrasion of the lumbar intervertebral disci（PLID）. Methods 72 patients corresponding to diagnosis standards, who were treated or hospitalized in our department, randomly divided into therapy group and control group, 36 cases per group. The control group received conservative treatment,besides that the therapy group was treated with psychotherapy based on patients＇ psychological obstacles.After 3-month follow-up, VAS scores and effective rate were compared between the two groups. Results The V AS scores of the therapy group at 3 days, 7 days and 14 days after treatment were （5.17±0.71）, （4.83 ± 0.74） and （3.52 ± 0.82） respectively, which were significantly lower than those in control group（6.13± 0.75）, （5.97 ± 0.76）,（5.24 ± 0.85）, （P〈0.05）, so the former was much better than the latter in symptom relief. The effective rates of the therapy group at 1 month and at 3 months were 91.7% and 88.9% respectively,while those of control group were 80.6% and 75.0%. The difference was significant （P〈0.O5）. It is obvious that the therapeutic effect of the therapy group was better than that of the control group. Conclusion Psychotherapy has the influence on the effect of conservative treatment for patients with PLID. Based on conservative treatment, psychotherapy can relieve low back pain and improve the effective rate significantly.
