摘要 目的观察5-氨基酮戊酸光动力疗法(ALA-PDT)治疗男性尿道口尖锐湿疣(CA)的疗效及治疗后局部病毒的清除情况。方法将入选的78例男性尿道口CA患者随机分为两组,治疗组42例,予以ALA-PDT治疗,对照组36例,予以高频电灼治疗,治疗后1周时判定疗效。并且在治疗前、后分别应用导流杂交基因芯片技术(简称HybrMiax)检测人乳头瘤病毒DNA(HPV-DNA)。结果治疗组和对照组的有效率分别为95.20%和100%,差异无统计学意义(P>0.05);治疗组(12.50%)与对照组(41.70%)的复发率、HPV清除率(分别为69.05%,22.22%)比较,差异均有统计学意义(P均<0.05)。结论 ALA-PDT治疗男性尿道口CA有效,复发率低,并且对尿道口局部的病毒清除作用优于传统的疗法。 Objective To find out the clearance rate of HPV and the recurrence rate of condyloma acuminatum of male urethral orifice with topical 5-aminolaevulinic acid photodynamic therapy(ALA-PDT).Methods Seventy-eight selected patients with condyloma acuminatum in urethral orifice were randomly divided into the ALA-PDT group and the control group.42 patients in ALA-PDT group were treated with ALA-PDT and the 36 patients in control group were treated with high frenquency electrocautery.The efficacy of the treatment was evalute one week later.The HPV-DNA were examined with diversion hybridization gene chip detection(HybrMiax)before and after treatment.Results Effective rates of treatment group and control group were 95.20% and 100% respectively,and there was no significant difference between them(P〉0.05).The recurrence of condyloma acuminatum was significantly decreased in the ALA-PDT group than that in control group(12.50% vs 41.70%)(P〈0.05).The clearance rate of HPV in the ALA-PDT group(69.05%)was significantly higher than that in the control group(22.22%)(P〈0.05).Conclusion ALA-PDT is an effctive and safe therapy with a low recurrent rate for condyloma acuminatum in urethral orifice of male.It's clearance rate of HPV is better than conventional treatment.
