人脐血单个核细胞移植治疗血管性痴呆的临床研究

Clinical study of human umbilical cord blood-mononuclear cell in transplantation for treating vascular dementia

目的观察人脐血单个核细胞经移植治疗血管性痴呆的疗效及安全性。方法采用自身对照研究，将脐静脉血单个核细胞输注入28例血管性痴呆患者体内，每例平均输注6份（细胞数1×10^8／份），每份间隔3-6d，监测细胞移植治疗前及治疗后6个月临床神经功能缺损程度（NFD）、简易智能精神状态检查量表（MMSE）和日常生活活动能力量表（ADL），评定其疗效，并进行安全性评价。结果人脐血单个核细胞移植后6个月，患者神经功能缺损程度评分较移植前明显降低（t=-12．31，P〈0．01）；简易智能量表评分和评价日常生活能力的Barthel指数均较移植前明显升高（拄17．82，t=21．02，P〈0．01）。在细胞移植过程中及移植后均未发生并发症及不良反应。结论人脐血单个核细胞经颈内动脉或手背浅静脉移植治疗血管性痴呆有较好的疗效，安全性好。

Objective To investigate the efficacy and safety of human cord blood mononuclear cells in transplantation for treating vascular dementia （VD）. Method The auto-matched method was adopted. Human cord blood mononuelear cells were injected into 28 patients with vascular dementia via the intracarotid or the superficial vein of the back of the hand, 6 samples were collected in each patient, cell number 1×10^8 per sample, with an interval of 3-6 days for each sample. Compared with pre-transplantation,the efficacies were assessed with neural function deficiency （NFD） and minimental state examination（MMSE） and activities of daily living（ADL） at 6 months following transplantation,and the safety after the treatment was also assessed. Results Compared with pre- transplantation, NFD were significantly decreased in patients at 6 months following transplantation（t=-12.31 ,P〈0.01）. MMSE and Barthel index grades（BI） were significantly increased （t=17.82,t=21.02,P〈0.01）. Complication or adverse reaction was not seen during and following transplantation. Conclusion Above-mentioned therapy is effective and safe to treat patients with vascular dementia.
