双水平无创正压通气对煤工尘肺肺心病患者心肺功能异常的影响研究 被引量:6

摘要 目的探讨双水平无创正压通气对煤工尘肺肺心病患者心肺功能异常的治疗效果。方法选取2008年1月—2010年12月于我院进行治疗的70例煤工尘肺肺心病患者为研究对象,随机分为对照组(常规治疗组,35例)和观察组(双水平无创正压通气组,35例),后将两组患者治疗前后的心肺功能进行检测及比较。结果观察组的肺活量占预计百分比(VC%)、最大通气量占预计值百分比(MVV%)、用力肺活量(FVC)、第1秒用力呼吸容积(FEV1)、FEV1/FVC、动脉血氧分压(PaO2)、动脉血氧饱和度(SaO2)及静脉血氧饱和度(SvO2)高于对照组,动脉二氧化碳分压(PaCO2)、平均肺动脉压(mPAP)及肺血管阻力(PVR)、心率(HR)低于对照组,差异有统计学意义。结论双水平无创正压通气可显著改善煤工尘肺肺心病患者心肺功能,值得推广应用。 Objective To study the effect of bilevelpositiveairwaypressure for improving abnormal cardiopulmonary functions of coal-worker′s pneumoconiosis with pulmonary heart disease.Methods 70 coal-worker′s pneumoconiosis with pulmonary heart disease in our hospital from Januar 2008 to December 2010 were selected as research object,and they were randomly divided into control group(routine group)35 cases and observation group(bilevel positive airway pressure group)35 cases,then the cardiopulmonary functions of the two groups before and after the treatment were detected and compared.Results The percentage of VC(VC%),percentage of MVV(MVV%),forced vital capacity(FVC),forced expiratory volume(FEV1),FEV1/FVC,partial pressure of oxygen(PaO2),arterial oxygen saturation(SaO2)and venous oxygen saturation(SvO2) of observation group were higher than those of control group,arterial partial pressure of carbon dioxide(PaCO2),mean pulmonary arterial pressure(Mpap),pulmonary resistance(PVR) and heart rate(HR) were lower than those of control group,CI was bigger than that of control group,there was significant differences.Conclusion The bilevel positive airway pressure can obviously improve abnormal cardiopulmonary functions of coal-worker′s pneumoconiosis with pulmonary heart disease,and it is worthy of spread and exploitation.
