黄芪对过敏性紫癜病儿肾损害的预防效果

PREVENTION OF ASTRAGALUS ON RENAL DAMAGE IN CHILDREN WITH ALLERGIC PURPURA

目的观察中药黄芪预防过敏性紫癜性肾炎(HSPN)的临床效果。方法将96例过敏性紫癜(HSP)病儿随机分为黄芪治疗组(50例)和对照组(46例)。两组给予相同的基础治疗,黄芪治疗组同时口服黄芪颗粒,每次3～4 g,每日2次,共8周。随访9～12个月,观察两组肾损害发生情况和HSP反复情况。结果黄芪治疗组随访期间11例(22.0%)发生HSP反复情况,明显少于对照组(χ2=4.156,P<0.05);黄芪治疗组HSPN发生率明显低于对照组(χ2=4.697,P<0.05);黄芪治疗组病儿HSPN出现时间为发病后(68.1±43.6)d,对照组为(115.8±49.1)d,两组间比较差异有显著性(t=2.193,P<0.05);对照组并HSPN者中53.3%的病儿发生在HSP发病后3个月内,而黄芪治疗组并HSPN者中71.4%的病儿发生在起病3个月内。两组病儿发生肾损害的程度比较差异无显著意义(P>0.05)。结论黄芪能有效减少HSP反复,预防或降低HSPN的发生,具有显著的肾保护作用。

Objective To evaluate the protection of astragalus on Henoch-Schonlein purpura nephritis（HSPN）.Methods Ninety-six children with Henoch-Schonlein purpura（HSP） were randomized to astragalus-treated group（50 cases） and control group（46 cases）.The same basic therapy was offered to both groups.For those in astragalus-treated group,astragalus particles were added（3 or 4 g/time,2 times a day） for 8 weeks.Followed up was conducted for 9-12 months.The damage of the kidney and recurrence of HSP were observed. Results Reoccurrence of HSP was found in 11 patients in the astragalus group（22.0%）,which was significantly lower than that of the control group（χ2=4.156,P0.05）.The incidence of HSPN of the astragalus group was significantly lower than that of the control（χ2=4.697,P0.05）.Moreover,the time of nephritis occurred in the astragalus group was（68.1±43.6） days after the onset of HSP,while that of the control was（115.8±49.1） days,being significant difference between the two groups（t=2.193,P0.05）.Furthermore,71.4% of HSPN in astragalus-treated group occurred within three months of HSP,while that in the control,53.3% appeared three months after the onset of HSP.The difference between the two groups was not significant in terms of the extent of renal damage（P0.05）. Conclusion Astragalus can effectively reduce the recurrence of HSP,prevent or decrease the occurrence of HSPN.
