全直肠系膜切除术治疗直肠癌的临床疗效观察

Clinical Effect Observation of Total Mesorectal Excision for Rectal Cancer

目的探讨全直肠系膜切除术治疗直肠癌的临床疗效。方法选择我院收治的40例肿瘤距肛缘均在10 cm以内的直肠癌患者,应用全直肠系膜切除术治疗的观察组患者20例,应用传统手术方法治疗的对照组患者20例,对两组术中失血量、平均手术时间、排尿、吻合口瘘、住院天数、局部复发率及性功能进行比较分析。结果两组在吻合口瘘、住院天数及1年、2年生存率的差异不显明,无统计学意义(P>0.05);在术中失血量、性功能障碍、局部复发率、泌尿功能障碍上有显著性差异,(P<0.05)。结论应用全直肠系膜切除术治疗直肠癌出血少、手术时间短、局部复发率和并发症发生率低,疗效确切,值得推广应用。

Objective Discussion on the clinical effect of total mesorectal excision（TME） for treatment of rectal cancer.Methods Select 40 cases of rectal cancer patients from our hospital with tumor within 10 cm from the anal margin,apply TME therapy on 20 patients as the observation group,apply conventional surgery on the other 20 patients as the control group,make comparative analysis on blood loss,average operation time,urination,anastomotic leakage,hospitalization time,local recurrence rate and sexual function between the two groups.Results It′s not obviously different in anastomotic leakage,hospitalization time,and 1-year,2-year survival rate,i.e.no statistical significance（P0.05）;while it′s obviously different in blood loss during surgery,sexual dysfunction,local recurrence rate,and urinary dysfunction with statistical significance（P0.05）.Conclusion Application of TME as a treatment for rectal cancer is featured with less bleeding,shorter operation time,low local recurrence rate and complication rate and exact effect,which should be widely promoted.
