摘要 目的探讨黏蛋白Mucin-3基因和多重耐药因子3(MDR3)在胆固醇结石形成中的作用及其相互关系。方法应用免疫组织化学法和逆转录聚合酶链反应(RT-PCR)技术,检测Mucin-3和MDR3在36例胆囊结石(结石组)、20例胆囊炎(炎症组)和12例正常胆囊(对照组)的胆囊黏膜和肝组织中的表达。结果结石组和炎症组胆囊黏膜Mucin-3表达阳性率分别为30.1%(11/36)和35.0%(7/20),显著低于对照组75.0%(9/12)(2χ=12.74、13.46,P〈0.01);结石组与炎症组之间比较,差异无显著性(χ2=0.21,P〉0.05)。结石组的肝组织中MDR3 mRNA的表达量低于炎症组和对照组,差异有显著性(F=19.785,q=8.432-10.037,P〈0.01);炎症组肝组织MDR3 mRNA的表达量与对照组比较差异无显著性(P〉0.05)。结石组胆囊黏膜中Mucin-3蛋白阳性表达者肝组织中的MDR3 mRNA的表达量与Mucin-3蛋白阴性表达者比较,差异有显著意义(t=3.142,P〈0.05)。结论 Mucin-3基因参与了胆囊结石形成的全过程。在胆囊结石形成的后期阶段,MDR3与Mucin-3基因相互作用,共同影响胆囊结石的形成。 Objective To investigate the roles of mucin gene(Mucin-3) and multidrug resistance 3(MDR3) in the formation of cholesterol calculus and their interaction. Methods Expressions of Mucin-3 and MDR3 in mucosa of gallbladder and liver tissue were detected via immunohistochemical method and reverse transcription polymerase chain reaction(RT-PCR) in 36 patients with cholelithiasis,20 with cholecystitis and 12 normal adults. Results Positive expression rates of Mucin-3 in mucosa of gallbladder were significantly lower in patients with cholelithiasis(30.1%) and cholecystitis(35.0%) than that in normal gallbladder(75.0%)(χ2=12.74,13.46;P〈0.01).However,the difference in positive expressions of Mucin-3 in mucosa of gallbladder was not significant between patients in the two groups(χ2=0.21,P〈0.05).In cholelithiasis,the expression of MDR3 mRNA in liver was lower than that in cholecystitis and controls(F=19.785,q=8.432-10.037,P〈0.01).The difference of MDR3 mRNA expression in liver tissue between cholecystitis and control group did not reach statistical significance(P〈0.05).Significant differences in positive expression of MDR3 mRNA in liver were found between the cholelithiasis patients with positive expression of Mucin-3 and those of negative expression(t=3.142,P〈0.05). Conclusion Mucin-3 participates in the whole course of the formation of cholecystolithiasis.In the later stage,the interaction of MDR3 and Mucin-3 affects the formation of the stone in gallbladder.
