摘要
目的了解多途径静脉置管留置情况,对非计划性拔管原因进行分析,提出防范措施,以达到按计划安全留置。方法收集我科近年364例行多途径静脉置管中非计划性拔管情况,进行原因分析。结果不同途径中心静脉置管非计划性拔管共67例(18.4%),分析原因主要为阻塞、机械性静脉炎、感染、血栓形成、导管异位、脱落、破裂。其中机械性静脉炎、血栓、感染形成与留置途径有关。结论化疗患者宜早期安置经各途径中心静脉置管。特别要根据患者具体情况扬长避短,精心选择不同的置管途径,加强各个置管及维护过程中的管理,非计划性拔管比例是可以控制下降的。
Objective To observe the vein remaining situation of multi-pathway central venous catheter and to analyze the reasons of unplanned extubation,then come up with the countermeasures to ensure safely planned extubation.Methods We collected 364 cases which had multi-pathway central venous catheter,and analyzed the reasons of unplanned extubation.Results There are 67 unplanned extubated cases,which occupied 18.4% of all patients.The main reasons are angiemphraxis,mechanical phlebitis,infection,thrombosis,ectopia or dropping off of the catheter.Mechanical phlebitis,infection and thrombosis were in relation to the catheter remaining pathway.Conclusion Chethomerapeutic patients should be inserted central venous catheter.According to their situations of vein,the best suitable vein should be chosen.Management of catheter insertion procedure and maintenance should be tightened up.Only doing so can unplanned extubation be controlled.
