良性淋巴组织中Ig/TCR基因重排分析

摘要 目的探讨良性淋巴组织Ig/TCR基因重排特点,为临床病理诊断提供更为可靠的依据。方法选取淋巴结反应性增生、扁桃腺炎、阑尾炎淋巴组织标本各20例和正常人外周血淋巴细胞标本15例,提取DNA,利用BIOMED-2引物系统进行Ig/TCR PCR扩增和核酸分子异源双链凝胶电泳分析。结果 75例均扩增出与BIOMED-2克隆分析系统规定的单克隆性重排大小和范围一致的产物,分别是IGH-A的320 bp片段和IGH-C的130 bp片段,其中IGH-A占77.3%,IGH-C占76.0%。所有样品均扩增出BIOMED-2克隆分析系统规定的非特异性重排产物,全部为IGH-E的100 bp片段和211 bp片段。结论 BIOMED-2系统存在一定的非特异性产物,主要集中在IGH-A、IGH-C、IGH-E;正确认识非特异性产物可为病理诊断和临床应用提供更为准确的依据。 Objective To discuss the characteristics of Ig/TCR gene rearrangements in benign lymphoid tissue,and to offer more reliable evidence for the clinicopathologic diagnosis.Methods DNA was extracted from lymphoid proliferations which included lymphoid proliferation tissues,tonsillitis tissues,appendicitis tissues,lymph tissues(20 each) and PBL(n=15).Ig/TCR PCR amplification and heteroduplex gel electrophoresis of nucleic acid were analyzed by BIOMED-2 standardized clonality analysis systems.Results In 75 samples,IGH-A 320 bp and IGH-C 130 bp according with the same size and range of positive monoclonal products were amplified in BIOMED-2 standardized clonality analysis system,among which,IGH-A accounted for 77.3% and IGH-C accounted for 76.0%.All samples were detected of IGH-E 100 bp and 211 bp which were regulated to aspecific rearrangement product in BIOMED-2 standardized clonality analysis system.Conclusion BIOMED-2 clonality analysis system has some nonspecific electrophoretic bands,including IGH-A,IGH-C and IGH-E.Understanding to the nonspecific electrophoretic bands can provide more reliable evidence for the pathological diagnosis and clinical application.
