摘要 目的:比较ACL ELITE PRO全自动血凝分析仪上Von Clauss法与PT衍生法(PT-der法)测定纤维蛋白原(FIB)的准确性及其临床应用价值。方法:用Von Clauss法与PT衍生法分别对新鲜混合血浆,20份正常质控品,301例临床样本和15例D-D二聚体阳性标本进行检测。结果:两种方法测定FIB的重复性良好;对20份正常质控品的测定结果差异无统计学意义(P>0.05);对临床样本按PT-der法检测结果分阶段做分析,FIB在1.51~3.50 g/L时,两种方法测定结果相近,差异无统计学意义(P>0.05),FIB>3.50 g/L时,PT-der法结果偏高,差异有统计学意义(P<0.05);15例D-D二聚体阳性标本用PT-der法测定结果明显高于Von Clauss法,差异有统计学意义(P<0.05)。结论:在ACLELITE PRO全自动血凝分析仪上,当PT-der法测出的FIB值在1.51~3.50 g/L时,直接报告结果,当PT-der法测出的FIB值>3.50 g/L时,以Von Clauss法复查的结果发出报告,两者结合应用可确保工作质量,提高实际工作效益。 Objective: To compare the accuracy and clinical application value of Von Clauss method and Prothrombin time derived fibrinogen determination method (PT-der method) on detecting fibrinogen on ACL ELITE PRO automatic coagula- tion analyzer. Methods: Using Von Clauss method and PT-der method to detect fresh pooled plasma, 20 normal control samples, 301 clinical samples and 15 D-dimer-positive samples, respectively. Results: The repetitiveness of the two methods on detecting fibrinogen is good. There was no statistical significance on the difference between the test results of the 20 normal control samples (P〉0.05). Dividing into different stages, using PT-der method to analyze the test results of the clinical samples: the level of fibrinogen is between 1.51 and 3.50 g/L, the results of the two methods were close, with no statistical significance (P〉0.05); while the level of fibrinogen is higher than 3.50 g/L, the result of the PT-der method was obvi ously high, with statistical significance (P〈0.05). For 15 D-dimer-positive samples, the result of the PT-der method was significantly higher than the Van Clauss method, with statistical significance (P〈0.05). Conclusion: On the ACL ELITE PRO automatic coagulation analyzer, when the level of fibrinogen detected by PT-der method is between 1.51 and 3.50 g/L, the result is reported directly. While the level of fibrinogen detected by PT-der method is higher than 3.50 g/L, the report is got by using Von Clauss to reexamine it. The combination of the two methods can make sure the quality of the work and improvement of the working efficiency.
