经岩乙状窦前小脑幕上下联合入路处理岩斜区巨大脑膜瘤1例

The combined presigmoid supra-infratentorial approach for the treatment of giant petroclival meningioma in 1 case

目的:介绍应用经岩乙状窦前小脑幕上下联合入路切除大型岩斜区脑膜瘤的方法和经验。方法:患者女性,57岁,头颅MR提示鞍区、左侧蝶骨嵴、岩斜区巨大肿瘤,大小为6.5 cm×6.5 cm×3.7 cm。手术通过左侧开颅经岩乙状窦前小脑幕上下联合入路切除肿瘤。结果:肿瘤次全切除,解除对于脑干及后组颅神经的压迫,患者恢复良好。结论:应用经岩乙状窦前小脑幕上下联合入路处理跨前中后颅窝的大型岩斜区脑膜瘤,可获得满意的手术疗效。

Objective： To introdace the method and experience of the combined preslgrnold supra-lntratentonal approach for the treatment of giant petroclival meningioma. Methods： Female patients, aged 57, head MR suggest that a giant tumor occupying the left tuberculum sellae, medial sphenoid ridge and petroelival, the size of 6.5 cm×6.5 cm×3.7 cm. Through the combined presigrnoid supra-infratentorial approach to remove the tumor. Results： A large part of tumor was removed, the brain stem and lower cranial nerve compression,the patient recovered well. Conclusion： The combined presigrnoid supra-infratentorial approach in the treatment of giant petroclival meningioma, will be satisfied with surgery.
