奈达铂联合氟尿嘧啶治疗晚期食管癌83例疗效观察

Nedaplatin Combined with Fluorouracil Treatment of Advanced Esophageal Cancer 83 Cases Observed

目的观察奈达铂（NDP）联合氟尿嘧啶（5-FU）治疗晚期食管癌的近期疗效和不良反应。方法将117例晚期食管癌患者分为3组,初治患者86例,NDP治疗A组52例,给予NDP联合5-FU治疗;顺铂（DDP）对照组34例给予DDP联合5-FU治疗;NDP治疗B组为经DDP治疗失败的31例复治患者,给予NDP单药治疗,每28d重复为1周期。2~3周期为1个疗程。化疗2周期后按WHO标准进行评价。结果共入组117例,均可以进行疗效评价,NDP治疗组和DDP对照治疗组的疗效接近,分别为51.92%和47.06%;经DDP治疗失败的复治患者有效率为25.81%;NDP治疗A组中骨髓抑制所致的血小板减少、白细胞减少的发生率较高,DDP对照组中恶心呕吐发生率较高,两组差异显著（P〈0.05）其他的不良反应两组相似。结论 NDP治疗晚期食管癌近期疗效与DDP接近,对DDP治疗失败的病例仍有一定疗效,其不良反应主要是骨髓抑制所致的血小板减少,白细胞减少。

Objective To nedaplatin（NDP） combined with fluorouracil（5-FU） chemotherapy in advanced efficacy and side effects.Methods117 patients with advanced esophageal cancer patients were divided into three groups,86 cases of newly diagnosed patients,NDP treatment A group of 52 cases,to give NDP combined 5-FU treatment;cisplatin（DDP） DDP control group of 34 patients were treated with combined 5-FU treatment;NDP treatment group B treated by DDP 31 patients with treatment failure in patients given monotherapy NDP,repeated every 28 days as a cycle.2-3 of 1 cycle of treatment.After 2 cycles of chemotherapy according to WHO criteria were evaluated.ResultsA total of 117 cases into the group,can effect evaluation,NDP treatment group and the efficacy of DDP in the treatment group close to,respectively,and 47.06% to 51.92%;by DDP in patients with treatment failure in retreatment rate was 25.81%;NDP treatment A group,bone marrow suppression induced thrombocytopenia,a higher incidence of leukopenia,DDP in the control group a higher incidence of nausea and vomiting,with significant difference（P0.05） other side effects were similar.ConclusionThe treatment of advanced esophageal cancer effect of NDP and DDP close to the case of DDP treatment failure is still a certain effect,its major side effect is bone marrow suppression induced thrombocytopenia,leukopenia.
