摘要
为研究乙型肝炎病毒(HBV)感染患者不同疾病发展阶段外周血IL-18的表达及其临床意义。以HBV感染相关性慢性肝病患者及健康人作为研究对象,采用夹心法酶联免疫吸附(ELISA)检测技术,对研究对象外周血IL-18进行检测,并用PCR-荧光定量检测研究对象血清中HBV DNA含量。结果显示,与正常对照组相比,HBV感染者血清IL-18水平显著增高(P<0.05);与肝硬化组及肝癌组相比,差异显著(P<0.05);而肝硬化组与肝癌组比较无显著性差别(P>0.05);低水平HBV DNA组IL-18水平显著低于中、高水平HBV DNA组(P<0.05);IL-18水平随HBV DNA、ALT、TBiL升高而升高(P<0.05)。表明细胞因子IL-18参与了HBV感染后的免疫应答,在HBV感染到肝癌的发展过程中可能起了一定作用,其表达水平与HBV DNA复制水平、肝功能损伤程度有一定的相关关系。
To investigate expression of cytokine IL-18 in peripheral blood of patients with hepatitis B virus infection in different stages of disease and to explore its clinical significance.the expression of IL-18 in sera of patients with HBV infection and the health adult individuals were compared by ELISA assay.For each patient,serum alanine transaminase(ALT),total bilirubin(TBiL) was measured biochemically and serum HBV DNA level was detected with fluorescent-quantitative PCR.Compared to the controls,it was found that the level of IL-18 in patients with HBV infection was increased(P0.05).The IL18-expression level in hepatic inflammation was higher than that of liver fibrosis and HCC(P0.05),but the expression level of IL-18 showed no statistically significant between liver fibrosis and HCC(P0.05).The expression level of IL-18 in the group A with low virus replication was lower than that of group B,C(P0.05).Correlation analysis showed that IL-18 had a positive correlation with ALT,TBiL,HBV DNA in the serum(P0.05).It suggests that IL-18 might join in HBV infection the immune system of HBV infection and play a role in the progression of hepatitis B virus infection.Its expression level appears to have significant correlation with the replication level of HBV-DNA and degree of liver damage.
