卒中后癫痫临床分析

Clinical Analysis of Postapolectic Epilepsy

目的探讨脑卒中后癫痫的临l床特点及发病机制。方法对1140倒脑率中病例中93例继发癫痫的临床资料进行回顾性分析。结果脑卒中癫痫的总发生率8．16％，卒中后早期癫痫的发生率为54．84％（51／93），晚期发生率为45．16％（42／93）。卒中后癫痫的发生率与病灶部位（皮质／皮质下）差异有显著性（P〈0．05）。结论脑卒中是老年人癫痫发作的常见原因，皮层痛灶较易发生癫痫。

Objective To probe the clinical features and the onset mechanism of postapolectic epilepsy. Methods The clinical data of 93 secondary epilepsy taken from 1140 cerebral apoplexies were studied retrospectively. Results The total incidence rate of post stroke acute symptomatic seizures and secondary epilepsy was 8.16%. Early and late onset rates of post-stroke seizures were 54.84% （51/93）＇and 45：16% （42/93） respectively. The incidence of the postapolectic epilepsy was quite different according to the location of focus （cortex/subcortex）. Conclusions Stroke is an important cause of acute symptomatic seizures and epilepsy in the elderly. The incidence of early seizure is higher in patients with cerebral isehemia, anoxia and hydrocephalus while cerebral cortical infarction is the ri~k factor for the occurrence of post-stroke epilepgy.
