微型钢板螺钉内固定治疗手部骨折的疗效分析

Treatment Analysis of Internal Fixation on Hand Fractures With Miniplate and Screw

目的分析微型钢板螺钉在内固定治疗手部骨折中的疗效。方法2000年1月-2008年6月，对64例156处手部骨折患者，应用切开复住微型铜板螺钉内固定治疗，术后早期功能锻炼。结果术后143例关节活动恢复良好，7例并发肌睫粘连，1例伤口感染，12例内固定在取出时螺帽断裂。无骨折移住、不愈合。随访3周-12个月，按照TAM评定法进行关节功能评价，优良率为92．3％。结论微型钢板螺钉治疗手部骨折，固定确切可靠，损伤小，术后可早期功能锻炼，手部骨折首选治疗方法。

Objective To study the treatment effect of miniplate and screw on hand fractures. Method Using miniplate and screw treated 156 fractures in 64 patients of metacarpal and phalangeal fractures from Jan. 2000 to Jun. 2008. Active motion was begun from second day postoperatively. Results 143 cases achieved well joint function, 7 cases occurred of adhesion of tendon and 1 case wound infection. The head of screw was broken on 12 cases when got out the internal fixation. No complications occurred of displacement and malunion of fractures. All cases were followed up 3 weeks to 12 months. Evaluated by the TAM system, the healing excellent rate was 92.3%. Conclusions Miniplate and screw have reliable and valid fixation and get less complications for hand fractures. Active motion of joints could be begun earlier postoperatively. Study showed that miniplate and screw were effective methods and should be chosen firstly.
