加强排痰对COPD患者急性加重期的治疗观察

The effectiveness of strengthen sputum elimination in patients with actute exacerbation of chronic obseructive pulmonary disease

目的探讨加强排痰对COPD患者急性加重期的影响。方法对83例COPD急性加重期患者进行排痰护理,其中38例作为观察组加强排痰,对照组45例只做常规排痰,对两组1周内平均每天排痰量、吸痰次数及1周后不吸氧情况下血气分析、FEV1预计值进行比较。结果观察组治疗前3d排痰量明显多于对照组(P<0.01),后4d排痰量及吸痰次数则少于对照组(P<0.01,P<0.05);在不吸氧的情况下,治疗7d后观察组血气分析及FEV1预计值明显优于对照组(P<0.05,P<0.05)。结论加强排痰对于COPD急性加重期患者改善通气、促进肺功能的恢复有明显的辅助治疗效果。

Objective To determine the effectiveness of strengthen sputum elimination in patients with acute exacerbation of chronic obstructive pulmonary disease（AECOPD）.Methods 83 patients with AECOPD were divided into observation group and control group.38 patients in the observation group accepted strengthen sputum elimination.While 45 patients in the control group were conducted with routine nursing intervention for expectoration.One week later,the daily sputum volume and sucking sputum number within the first week,the blood gas without oxygen inhalation and the predicted value of forced expiratory volume in one second（FEV1%）were compared.Results Daily sputum volume increased obviously（P0.01） in the first 3 days,the daily sputum volume and sucking sputum number decreased notably（P0.01,P0.05） in the last 4 days,the blood gas analyzed index without oxygen inhalation and the FEVI%increased significantly（P0.05,P0.05）in the observation group compared with the control group.Conclusion Strengthen sputum elimination can improve ventilation and facilitate the recovery of pulmonary function in the patients with AECOPD,the positive function on treatment of AECOPD.
