涎腺肿瘤冰冻切片诊断

Experience in frozen section histopathology for salivary gland tumors

目前,涎腺肿瘤最终手术方案的制定主要依赖术中组织冰冻切片诊断。对于病理医生来说,术中组织冰冻切片诊断的要求是确定涎腺肿瘤的良、恶性以及恶性程度,而并不一定要明确肿瘤的具体类型。观察肿瘤的生长方式,即肿瘤有无包膜、是否呈浸润性生长以及浸润状况是判断涎腺肿瘤良恶性的主要依据之一,对于某些类型涎腺肿瘤来说,浸润性生长可能是支持其为恶性的唯一证据。熟悉临床资料,做好组织冰冻巨检和取材工作,保证组织冰冻制片质量均有助于保证及提高组织冰冻切片诊断准确性。

For salivary gland neoplasias,generally,intraoperative frozen section has great value in helping to optimize surgical treatments.Accurately separating benign salivary gland tumors from malignancies and tumor grading are more important than tumor classification in frozen section.The accurate diagnosis of benign salivary gland tumor or malignant one usually depends on evaluation of the interface with surrounding tissues.For some tumor entities,the infiltrative growth pattern may be the only evidence to support the diagnosis of malignancy.Being familiar with the clinical data,good sampling of lesional tissue,and high quality frozen slices may be helpful in ensuring and improving the accuracy of frozen section for salivary gland tumors.
