胰腺实性假乳头状瘤的超声诊断与临床分析

Ultrasonographic diagnosis and clinical analysis of solid pseudo-papillary tumor of pancreas

目的探讨胰腺实性假乳头状瘤（SPTP）术前检查的超声表现及临床特点。方法回顾性分析经手术病理证实的15例胰腺实性假乳头状瘤患者的术前超声检查结果和临床资料。结果 15例SPTP患者中,男性13.3%（2/15）,女性86.7%（13/15）;年龄18~31岁,平均（24.3）岁,中位23岁。肿瘤大小平均为5.0 cm（3.3~7.5 cm）,肿瘤位于胰头部6例,胰体部6例,胰尾部3例。单一超声检查诊断准确率为33.3%（5/15）,结合年龄、性别,肿瘤标志物指标后,诊断准确率为66.7%（10/15）。结论胰腺实性假乳头状瘤有其特征性的超声表现及临床特点,两者结合能明显提高术前诊断准确率。

Objective To review the ultrasonic findings and clinical manifestation of solid pseudo-papillary tumor of pancreas（SPTP） before operation.Methods Both ultrasonic imaging findings and clinical data of 15 case of SPTP were retrospectively analyzed.Results There were 13 female and 2 male patients in this study,aged 18~31y（mean 24.3y）.The average size of the tumor was 5.0 cm（3.3~7.5 cm）,and 6 case positioned in head of the pancreas,6 case in the body,3 case in the tail.The diagnostic accordance rate was 33.3%（5/15） by ultrasonographic examination alone,it was raised to 66.7%（10/15） by combination of age,sex and tumor marks.Conclusion Based on distinctive ultrasonic manifestation and clinical characteristics the diagnostic accordance rate of solid pseudo-papillary tumor of pancreas can be improved markedly.
