斑点追踪成像技术评估赫赛汀治疗患者左室收缩功能的初步研究 被引量:5

摘要 目的应用斑点追踪成像技术评估赫赛汀治疗患者左室收缩功能。方法赫赛汀联合紫杉类化疗药治疗的乳腺癌患者34例,同期单用紫杉类化疗药治疗的乳腺癌患者40例,获取其二尖瓣水平、乳头肌水平及心尖水平左室短轴观及心尖位左长轴观、四腔观、二腔观图像,应用斑点追踪技术测量并比较各节段室壁运动的收缩期峰值应变。结果长轴方向和短轴方向赫赛汀治疗组患者收缩期大部分节段室壁最大圆周应变及径向应变明显小于非赫赛汀治疗组患者对应节段。结论使用赫赛汀治疗的患者大部分节段室壁收缩期峰值应变减低,超声斑点追踪成像技术能更早地评估赫赛汀治疗的患者左室功能,以指导临床及时采取干预措施。 Objective To evaluate systolic function of left ventricle in patients receiving Herceptin by two-dimensional strain echocardiography.Methods Thirty-four patients receiving Herceptin and Paclitaxel/Taxotere and forty patients receiving Paclitaxel/Taxotere alone were included.High frame rate two-dimensional images were recorded from the left ventricular short—axis views at the levels of mitral annulus,papillary muscle and apex,and the apical four-chamber view,two-chamber view and long-axis of the left ventricle.The peak systolic longitudinal strain were measured in apical views,the peak systolic circumferential strain and radial strain were measured in the left ventricular short-axis views using 2D strain software.Results On long-axis and short axis,most parameters of patients receiving Herceptine and Paclitaxel/Taxotere were significantly lower than those receiving Paclitaxel/Taxotere alone.Conclusions The systolic longitudinal strain,circumferential strain and radial strain of left ventricular regional myocardium are impaired in patient receiving Herceptin.Speckle tracking echocardiography can be used as a new method to evaluate the left ventricular systolic function quantitatively.
