手术治疗椎管占位无神经损伤胸腰椎爆裂性骨折临床疗效探讨

目的：探讨椎管占位无神经症状的胸腰段爆裂性骨折手术治疗效果。方法：对28例确诊为椎管占住无神经症状的胸腰椎爆裂性骨折患者采用后路内固定植骨技术治疗。结果：本组28例平均随访12个月，术后X线片、CT显示复住、固定良好。椎体内、外植骨均获得骨性融合，Cobb’s角由术前10°～25°恢复到术后7°～15°，椎体前缘高度由术前50％恢复到术后的90％。术后均未出现神经功能障碍。结论：于无神经损伤的胸腰椎爆裂性骨折，不管椎体压缩严重程度均应早期手术。因为通过手术可以维持脊柱三维空间结构，通过对伤椎植骨减轻椎管占位现象，后路手术近路，保留了脊柱后路结构，手术操作简单、创伤小、恢复快，适合推广使用。
