运动试验对冠心病的诊断价值

Diagnosis Value of Treadmill Exercise Test on the Coronary Heart Disease

目的评价平板运动试验(TET)对冠心病的诊断价值。方法选取我院临床拟诊为冠心病患者84例,行平板运动试验后1个月内行冠脉造影(CAG),以CAG检查结果为诊断冠心病的"金标准",对TET结果进行分析。结果平板运动试验诊断冠心病敏感性76.0%、特异性67.6%、准确性72.6%、假阳性32.3%、假阴性24.0%,其中多支病变平板运动试验阳性率较单支病变高(P<0.01)。结论 TET诊断冠心病的敏感度和特异度较高,能够较准确地评估冠脉病变的严重程度,判断患者的治疗效果及预后,具有安全、无创、价廉、操作简单等优点。

Objective To evaluate the treadmill exercise test（TET）for the coronary heart disease. Methods 84 patients with coronary heart disease were given treadmill exercise test,and then coronary angiography （CAG）in a month. CAG test results were＂gold standard＂in the diagnosis of coronary heart disease,and TET results were analyzed. Results The sensitivity of treadmill exercise test in the diagnosis of coronary artery disease was 76%,specificity was 67.6%,accuracy was 72.6%,false positive was 32.3%,false negative was 24%. The positive rate of treadmill exercise test in multi-vessel disease was higher than the single-vessel disease（P〈0.05）. Conclusion The sensitivity and specificity of TET in the diagnosis of coronary heart disease was high. TET that was safe,noninvasive,inexpensive,easy to operate,could more accurately assess the severity of coronary artery disease,determine treatment and prognosis of patients.
