顽固性鼻出血鼻内镜下双极电凝止血疗效分析

Hemostasis efficacy analysis of bipolar in stubborn epistaxis treatment under nasal endoscopy

目的:探讨对顽固性鼻出血患者行鼻内镜下双极电凝止血的疗效。方法:将顽固性鼻出血患者84例随机分为两组。对照组42例采用传统鼻腔凡士林油纱条等填塞止血+药物治疗,实验组42例应用鼻内镜下双极电凝止血+药物治疗。结果:实验组术中止血迅速、彻底,术后鼻腔黏膜变化不明显,很少发生鼻腔再出血现象,术后综合征反应发生率低,住院时间短。两组比较差异有统计学意义(P<0.01)。结论:鼻内镜下双极电凝止血,能直视下寻找出血点,大多能明确出血部位,减少鼻腔填塞止血盲目性,其临床止血疗效确切,操作简单,创伤小,痛苦小,并发症少,病程短,治愈率高,值得临床广泛应用。

Objective： To discuss the hemostasia efficacy of bipolar used in epistaxis treatment under nasal endoscopy.Methods： 84 patients who had obstinate epistaxis were divided into two groups.42 cases in the control group,using traditional medication as well as hemostasis with packs such as petroleum jelly and meche.While 42 cases in the experiment group,using bipolar treatment under nasal endoscopy and medication.Results： In the experiment group,hemostasia was quick and thorough during the surgery.After the surgery,the changing of the mucous membrane of nose was not obvious,and we scarcely found any symptom of rehaemorrhagia in nasal cavity.They had low syndrome rate after the surgery meanwhile less time being hospitalized.So the treatment had a significant meaning in statistics by comparing with the two groups（P0.01）.Conclusion： The hemostasis efficacy of bipolar used in epistaxis treatment under nasal endoscopy,can be used to find the blutene chloaide orthoptically,locate the bleeding part mostly,and decrease the blindness of hemostasis with packs.It has many advantages such as exact clinical curative hemostasia effects,simplicity of operator,slight trauma,less pain and complication,short course of diseases and highcure rate.The method is worthy of clinical use.
