双极人工股骨头置换术后中远期疗效分析

An Analysis on Long term Results of the Replacements of Bipolar Prostheses.

目的：分析双极人工股骨头置换术后中远期疗效。方法：１９８８ ～１９９５ 年间施行双极人工股骨头置换术１８４ 例，取得随访７５ 例，平均随访时间６ 年４ 个月。评估临床疗效，并摄 Ｘ 线片观察髋臼磨损和假体松动情况，动态测量假体内、外关节活动度。结果：临床疗效满意率达８５３ ％ 。髋臼磨损与假体内关节活动消失和头臼不称有关。假体不稳可导致大腿疼痛。结论：双极人工股骨头置换术是高龄股骨颈头下型骨折患者的一种较好的治疗选择。

Objective:To investigate the long term results of the replacement of bipolar prostheses.Method:184 cases had had performed by replacements of bipolar prostheses from 1988 to 1995.Among these cases,75 patients(40 7%) were followed up after surgery a mean period of 76 months.Clinical results evaluation were performed,the changes of acetabulum and prosthetic stem were assessed with the radiograph.The motion of bipolar prosthetic inner bearing and outer bearing were measured by A P X ray plane of hips adduction and abduction.Result:The clinical satisfactory rate is 85 3%.The acetabulum erosion was related with the disappearing of the inner bearing movement,and the inappropriate match between the prosthetic cup and acetabulum.The thigh pain was caused by instability of prosthetic stem.Conclusion:The bipolar hemiarthroplasty is an effective method for the elderly patient with femoral neck fracture.
