摘要
目的探讨宫颈癌病理学类型与人乳头瘤病毒感染型别(HPV-16和HPV-18)的关系。方法每例宫颈癌标本取样两份,一份进行组织病理学检查,包括HE、PAS和粘液卡红染色;另一份用于提取DNA,分别用HPV-16和HPV-18基因组探针进行核酸杂交。结果216例宫颈鳞癌分为角化型、大细胞型和小细胞型三种类型;41例宫颈腺癌分为粘液性腺癌、腺鳞癌和宫内膜样腺癌三种类型。核酸杂交结果表明,宫颈鳞癌中,HPV-16的感染率较高,HPV-18的感染率较低,二者比较差异有显著性(P<0.01);宫颈腺癌中,HPV-18的感染率较高,HPV-16的感染率较低,二者比较差异有显著性(P<0.01);不同类型的宫颈鳞癌之间比较,HPV-16的感染率差异无显著性(P>0.05);不同类型的宫颈腺癌之间比较,HPV-18的感染率差异无显著性(P>0.05)。结论宫颈鳞癌与HPV-16感染密切相关,宫颈腺癌与HPV-18感染密切相关;而宫颈鳞癌中的不同类型与HPV-16之间以及宫颈腺癌中的不同类型与HPV-18之间均无明显的相关性。
Objective To study the relationship between histopathological type of cervical carcinoma and human papillomavirus type. Methods Each specimen of from cervical carcinoma were divided into two portions. the one was sectioned and stained respectively with HE, PAS or Mayer's dyeing; the another was subjected to DNA extraction and followed by hybridizization with respectively HPV-16 or HPV-18 DNA probe. Results 216 cases of cervical squamous cell cacinoma were classified into three types,namely: keratinized equamous cell carcinoma, large squamous cell carcinoma and small squamous cell carcinoma: 41 cases of cervical adenocarcinoma were classified into three type, namely: mucinous adenocarcinoma, mixed adenosquamous carcinoma and endometrioid adenocarcinoma. The infection rate of HPV-16 was higher than that of HPH-18 in cervical squamous cell carcinoma, there was significant difference between the 2 virus types(P<0. 01); the infection rate of HPV-18 was higher than that of HPV-16 in cervical adenocarcinoma, there was significant difference between the 2 virus types (P < 0.01); there was no difference between the different histopathological types of cervical spuamous cell carcinoma and HPV-16, the same between cervical adenocarcinoma and HPV-18 (P > 0. 05 ).Conclusion Cervical squamous cell carcinoma was strongly correlated with HPV-16, cervical adenocarcinoma was obiviously correlated with HPV-18; the different histopathological types of cervical squamous cell carcinoma or cervical adenocarcinoma was not correlated with HPV-16 or HPV- 18.
