溃疡病穿孔Ⅰ期胃大部切除手术指征探讨

Investigated operation indication of acute subtotal gastrectomy of peptic perforated ulcer

目的探讨消化性溃疡病急性穿孔行急诊Ⅰ期胃大部切除手术的手术指证。方法回顾性分析1980年至1997年6月消化性溃疡病急性穿孔行急诊Ⅰ期胃大部切除手术56例病例。结果手术近期并发症发生率8.9％，皆保守治疗痊愈出院，无死亡。结论总结手术指征为:①年龄＜55a;②穿孔时间＜24h;③腹腔液体＜1000ml;④有溃疡病穿孔、出血、梗阻或不能除外癌变病史;⑤穿孔时伴有出血、梗阻或不能除外癌性穿孔;⑥多发性或复合性溃疡;⑦胼胝性溃疡穿孔;⑧没有休克或内科疾病。

Objecive To discuss the operation idication of the acute perforation from gastroduenal ulcer by acuter subtotal gastretomy. Methods We reviewed 56cases of the acute perforation from 1980 to June 1997. Results The early mploperative complication morbidity was 8. 9% through the conservation treatment recovery out of hospital, no mortality. Conclusion The summed up operation indication was: ①age< 55 years old; ②perforation time< 24h; ③the liquit in the abeonminal cavity<1000ml;④ followed Perforated. bleeding. obstruction or no boldes carcinomatous ulcer in history; ⑤bleding.obstruction or no besides caicinomatous ulcer in perforation; ⑥multipal and compound ulcer; ⑦callour perforated ulcer; ⑧no followed shock and meidical disesses.
