围生期心肌病33例临床分析

Clinical analysis of 33 cases with peripartum cardiomyopathy

目的探讨围生期心肌病的临床特点。方法回顾性分析33例围生期心肌病病例的心电图,胸片,超声心动图等指标及治疗转归。结果 33例患者均有不同程度的心力衰竭表现,入院时心功能(NYHA分级)III级16例,IV级17例,其中31例患者左室射血分数平均值(0.46±0.09)。经过治疗后,患者临床症状缓解或消失,其中显效11例,有效21例,无效1例,其中7例产前发病患者采取适时终止妊娠,母婴平安。结论围生期心肌病经过及时的诊断和治疗,部分患者心功能可以得到改善和恢复。

Objective To summarize the clinical features of peripartum cardiomyopathy.Methods 33 patients diagnosed as PPCM in our hospital from Jan 2003 to Jul 2010 were studied retrospectively.All of their clinical manifestation,EKG change,chest X-ray and echocardiogram feature,treatment and outcome were analyzed.Results All PPCM patients were shown with heart failure,in which 16 cases with Class III（NYHA cardiac function classification）and 17 cases with Class IV.Averaged left ventricular ejection fraction of the 31 patients was 0.46±0.09.Among all patients,11 cases were effectual,21 cases were effective and 1 case was invalid.Meanwhile,7 patients deliveried safely when pregnancy terminated timely.Conclusions The outcome of PPCM patients could be improved with early diagnosis and treatment.
